Registering a Transferring Student
Please come to the Administration Center as soon as possible.
These items are necessary for school registration:
1. Proof of residency within Orland School District 135 boundaries from both Category A and Category B (as described below). The parent's name and address must appear on the documents and they must be current. Acceptable proof is as follows:
Category A (one document required)
- Most recent home property tax bill
- Current month mortgage statement
(If home closing just took place, HUD I Settlement document)
- Signed and dated 12-month lease with two rent receipts
- If parent is not the homeowner: notarized affidavits of residency from the resident owner and the resident custodial parent along with supporting documents from both Category A (owner) and Category B (parent)
As Needed to Prove Residency
(If parent is not the homeowner)
Category B (two documents showing proper address are required)
- Driver's license / State ID
- Cable, phone (not cell), or credit card bill
- Public aid card
- Homeowner's insurance premium receipt
- Gas, electric, or water bill
- Vehicle registration
2. Transfer slip from the latest school showing school and grade attended. In IL, this is the Illinois State Transfer Form.
3. Name and address of the student's previous school. This is the full and complete address to which a records release form will be sent.
4. Student's birth certificate. The student's government-certified document, not a hospital commemorative card.
5. Check, money order or credit card (Visa, MC, Discover) for the 2011-12 school year student registration fees (cash not accepted): $133 for kindergarten, $160 for Grades 1-5, $172 for Grade 6, and $167 for Grades 7-8. There is an additional cost for a calculator for Grades 1-5 ($5) and for the Grades 6-8 gym suit ($15) and calculator ($15).
6. Student's health records. A copy of the last physical exam including immunization dates should be provided. If a child is transferring from outside of Illinois, a new physical exam and an eye exam is required. All health forms are to be returned to your child's school to the attention of the nurse.
7. If a child has special needs, any records necessary for a special placement.
8. Any student custodial agreements, decrees, judgments, or other documents if applicable.
