Just a reminder that physical and dental examinations are required for all Kindergarten and transfer students who are new to District 135. Students will be excluded from school if the physical has not been completed or if there are missing immunizations. If you still need to get the required immunizations for kindergarten, or if you have been contacted regarding missing immunizations on you child's medical form please call your school nurse. Parents will be contacted if any problems are noted. Please call the nurse if you are having any health concerns regarding you child.
Illinois State law requires that all children entering school for the first time and all sixth graders must present evidence of a recent physical examination and of immunizations against certain diseases. Proof of an eye examination by an optometrist or physician who provides complete eye examinations must be submitted to the school for all newly enrolled students as well. A dental examination is mandated for kindergarten, second and sixth grade students. The vision forms, physical examinations and immunization records must be turned into the school nurse's office by the beginning of the school year. Dental examination forms are due by May 15th of the school year.
Screening for Vision and Hearing takes place in the fall.
Food Allergies

Administration of Medication
Medication can be administered only with a signed physician's order, which is part of the District 135 medication form, which can be obtained in the nurse's office, or downloaded here. This policy is inclusive of both prescription and over the counter medications. Medications must be brought to school by the parent or guardian in the original, properly labeled container. Only the nurse or a school administrator can administer medicine to a student.
